IPM Associates, Inc. - Class & Workshop Registration Forms

If you are interested in attending any of the the following classes, you may register a number of ways. Print out the form you need, complete it by hand or type, then mail it to our Address or fax it to us. Both the Fax Number and our Office Address are found at the top of each Registration Form and on our Contact Us page. There are two ways to print the form you need. Select either HTML or PDF format and then print it on your printer. Fees for the classes can be paid via mail, at the door on class day, or if a Purchase Order is necessary just give us the P.O. Number on the Registration Form and we will send an invoice. Be reminded, all classes are by registration only and space in some is limited, so register as early as possible.

.PDF files are cross-platform files that can be viewed and printed on any computer that has the free Acrobat Reader software installed on it. Acrobat Reader can be downloaded for free from Adobe.

To download and print these forms, simply click on any .PDF link in this page and it will be downloaded to your computer. It should automatically open and display in your Acrobat Reader plugin. From there you can print and/or save it as desired. If the file is not opened up in Acrobat Reader automatically, right-click on the desired .PDF link and "Save Target as..." the file to a location your computer and then open the .PDF file with Acrobat Reader manually.
